Interest form vs application: what's the difference?
An interest form is the first step: who they are, how to reach them, and what they want to help with. A volunteer application is what you collect after you decide to move someone forward: references, emergency contact, role documents, and later consents. Do not put both on the same first page.
What happens after someone submits?
You get their contact details and interests so you can follow up. Save the longer application for people you want. You follow up by email or phone yourself.
Where to put it
Published forms can be hosted at /o/your-org/volunteer or embedded with a script tag. For shift sign-ups, use WhoCanBeThere, a sister product. This form does not schedule volunteers.

Should an interest form ask for date of birth?
No. Date of birth is personal information. Ask it after someone has shown interest, not on the first form.
Does this form schedule volunteer shifts?
